29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) 500 Chase Road, Shavertown, PA: Employees were not protected from falling by a guardrail system with toeboards, safety net systems, or personal fall arrest systems while performing roofing construction activities from a 4:12 pitch commercial roof, exposing employees to a fall up to fifteen (15) feet to the ground level, observed on or about August 6, 2019.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 500 Chase Road, Shavertown, PA:  Employees were using an extension ladder to gain access to the roof with the ladder side rails extended less than three feet about the landing surface, on or about August 6, 2019.  ABATEMENT CERTIFICATION REQUIRED
