29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  (a) Continental Rollo-Mixer Mark V: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language that would satisfy the requirement for a sign was not provided to inform affected employees of the hazards associated with entry into this permit required confined space mixer.

29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present:  (a) Reticulation room - Reticulatus 3000: On or about August 7, 2019, in the Reticulation room, a Class 1, Zone 1 environment, electrical motors located on the reticulation chamber were not rated for the environment.  This exposed employees to hazards associated with flammable hydrogen gas.
