29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface:    Production Area - On or about August 7, 2019, a section of the top edge height of top rail surrounding the conveyor belt system was damaged not allowing the proper vertical height from the floor, thereby employees were exposed to the hazard associated with falls.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s)    a. Production area - On or about August 7, 2019, employees were allowed to operate a Caterpillar #35 (serial no. AT14E00161) powered industrial truck that did not have a functioning horn, thereby exposing employees to the hazards associated with a moving forklift truck.    Allied Metal Company was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.178(q)(1), which was contained in OSHA inspection number 1132531, citation number 1, item number 1, and was affirmed as a final order on June 7, 2016, with respect to a workplace located at 4528 W. Division Street, Chicago, Illinois 60653.
