29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) Top of the roof, East Wall: On or about August 8, 2019, the employer exposed employees to back injuries while installing roofing material, in that the employees attached life-lines to carabiners located in the front webbing of the body harness located at their waist.
