On and about August 6, 2019, employees reroofing a single story house were exposed to multiple hazards, including but not limited to, falls, struck by, and electrical when they had not been trained to recognize and avoid or correct hazards or to understand and follow the OSHA Standards that applied to their scope of work.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On and before August 6, 2019, employees performing roofing work had not been trained to recognize and avoid fall hazards or to understand and follow the OSHA Standards that applied.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  On and about August 6, 2019, employees using an extension ladder to access and egress the roof of a single story house had not been trained to properly use ladders or understand the OSHA Standards applicable to their scope of work.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    On August 6, 2019, employees were exposed to falls while reroofing a single story house without fall protection.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    On August 6, 2019, employees were exposed to falls while accessing and egressing the roof using an extension ladder that did not extend at least 3' above the upper landing.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  On and about August 6, 2019 employees were exposed to shock hazards at a worksite located at 2613 Greenbrier Rd. Green Bay when damaged extension cords were available for use.
