29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Jose Arco's Jr. Welder Company at 3009 West Russell Street, Sioux Falls, SD 57107:  On or about and at times prior to August 8, 2019, the employer did not ensure that employees that were accessing an upper balcony were protected from fall hazards greater than six feet.  This condition exposed an employee to a fall hazard greater than six feet.

29 CFR 1926.351(b)(4): Welding cables in use were in need of repair:  (a) Jose Arco's Jr Welder Company at 3009 West Russell Street, Sioux Falls, SD 57107:  On or about and at times prior to August 8, 2019 the employer did not ensure that welding cables that were being used for installation and welding of the steel beams were free from damage  This condition exposed employees to shock hazards.
