The employer failed to train and evaluate the employee's performance to work safely and in close proximity to an RA Switch.

On or about 08/05/2019, Location: Residential Condominium East's electrical vault at 400 Kings Point Drive, Sunny Isles Beach, Florida, 1) An employee was exposed to electrical hazards while installing a cable from two new transformers to a main cabinet without de-energizing the main electrical switch, wearing regular steel toed boots without EH rating. The employer failed to provide the employees with steel toed safety shoes with EH rating, as per employer's footwear policy, at no cost.
