29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 08/12/19, on roofing jobsite at a 2-story house at 97 Briscoe Avenue, Buffalo, NY: An employee engaged in roofing work on the steep roof on the right side of the house was exposed to a fall of approx. 18 ft.  No fall protection system was being used. Pitch of the roof was approx. 12 in 12.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 08/12/19, on roofing jobsite at a house at 97 Briscoe Avenue, Buffalo, NY:  Employee used an extension ladder to access the roof on the right side of the house. The siderails of that extension ladder only extended approx.  20 inches above the edge of the roof.      NO ABATEMENT CERTIFICATION REQUIRED
