29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a)  222 Ridge Avenue Waynesboro PA - An employees was observed performing roofing work activity on the main roof with a 10 in 12 pitch without the benefit of a fall protection system, thereby exposing employees to a fall from the main roof to the porch roof of approximately 8 feet to the porch roof below, on or about August 6, 2019.     (b)  222 Ridge Avenue Waynesboro PA - An employees was observed performing roofing work activity on the porch roof with a 4 in 12 pitch without the benefit of a fall protection system, thereby exposing employees to a fall from the porch roof of approximately 10 feet 8 inches to the ground below, on or about August 6, 2019.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  222 Ridge Avenue Waynesboro PA - Employees were using an aluminum extension ladder to access the porch roof from the ground, the ladder did not extend three feet above the landing, thereby exposing the employees to injuries from ladder collapse or deflection, on or about August 6, 2019.  (b)  222 Ridge Avenue Waynesboro PA - Employees were using an aluminum extension ladder to access the main roof from the porch roof, the ladder did not extend three feet above the landing, thereby exposing the employees to injuries from ladder collapse or deflection, on or about August 6, 2019.   Abatement certification and documentation required within 10 days after abatement date.
