29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Building Exterior side roof: An employee was exposed to fall hazards of approximately 9 feet while engaged in roofing activities on a 10/12 steep roof without means of fall protection in place, on or about 8/13/2019.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Building Exterior: An employee was exposed to a fall hazard while accessing an extension ladder of a residential home. The ladder did not extend at least 3 feet above the upper landing surface, on or about 8/13/2019.
