29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers: a) Job site, 1816 A Street, Pittsburgh, PA: On or about August 2, 2019, during construction activities, the employer did not ensure that an appropriate system was implemented at the site to identify and correct safety and health hazards. Deficiencies noted during the inspection included, but were not limited to, abundant levels of construction debris laying in the walking/working path and on stairways, lack of handrails on stairways and electrical cords laying under construction debris that were subjected to physical damage.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Job site, 1816 A Street, Pittsburgh, PA: On or about August 2, 2019, during construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices while in a dumpster, exposed to a fall height of approximately 6 feet 3 inches. b) Job site, 1816 A Street, Pittsburgh, PA: On or about August 2, 2019, during construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices while on the second floor of the building where floor holes existed, exposing employees to fall hazards of approximately 10 feet. c) Job site, 1816 A Street, Pittsburgh, PA: On or about August 2, 2019, during construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices while on the second floor of the building where window openings existed, exposing employees to falls hazards of up to approximately 14 feet.
