29 CFR 1926.502(e)(8)(i):  Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged directly to webbing, rope or wire rope.  On or about: 7/10/2019  Location: West Roof end    a)  An employee engaged in roof work used snaphook connector attached directly to rope when the snaphook connector was not designed for such connection exposing an employee to a fall hazard of approximately 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about and times prior to: 7/10/2019  Location: West end of roof  a) Employees were engaged in roof work at approximate of 20 feet. The employer did not extend the aluminum extension ladder at least 3 feet above the upper landing surface to assist employees mounting and dismounting the ladder.
