29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Location: Front side of the house on the home entrance roof @ 14 Church St. Berkley, MA. 02779    On or about 8/19/19, an employee working on from a roof was exposed to fall hazards of 12 feet to the ground below, where a guardrail system or, personal fall arrest system was not in use.

29 CFR 1926.1060(a)(1): The employer did not ensure that each employee has been trained by a competent person in the following areas, as applicable:    Location: Front side home entrance roof of existing residential home.    On or about 08/19/19 the employer did not have a ladder training program nor were employees trained on ladder use where an employee fell 12 feet to the ground below and sustained injuries while working.

29 CFR 1904.39(a)(3): The employer did not report the fatality, in-patient hospitalization, amputation, or loss of an eye using one of the following methods: (i) By telephone or in person to the OSHA Area Office that is nearest to the site of the incident. (ii) By telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742). (iii) By electronic submission using the reporting application located on OSHA's public website at www.osha.gov.    Location: Front side of home on the 10 pitch roof of an existing residential home.    On or about 08/19/19 an employee fell and sustained injuries and was hospitalized.     THE EMPLOYER DID NOT NOTIFY OSHA ABOUT AN EMPLOYEE HOSPITALIZATION.
