On or about 08/19/19 the employer did not have a ladder training program nor were employees trained on ladder use where an employee fell 12 feet to the ground below and sustained injuries while working.

On or about 08/19/19 an employee fell and sustained injuries and was hospitalized. THE EMPLOYER DID NOT NOTIFY OSHA ABOUT AN EMPLOYEE HOSPITALIZATION.
