29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Truck body and chassis (which includes the Knapheide truck body attached to the motor driven truck vehicle) were worked on by employees, which included work underneath the vehicle, without de-energizing the vehicle. Keys may or may not have been removed or were laid on the console, and emergency brakes may or may not have been set during the work. A role over or struck by hazard existed, such as but not limited to, movement of the vehicle from unexpected start-up. Although a non-formal procedure may have existed for some employees, such as removing keys from the ignition, honking the horn before driving, and checking for someone still under the vehicle before driving, it was not consistently applied and it was not documented and implemented as required by 1910.147. Abatement documentation is required for this violation.
