29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  612 Douglas St, Uxbridge, MA  On August 20, 2019 and at times prior, employees operated telehandler lifting vehicles with only on-the-job training instead of the formal training detailed in 29 CFR 1910.178(l)(2)(ii) that training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  On August 20, 2019, an untrained operator employee tipped over a telehandler onto its side after misjudging the effect of transporting an elevated load over uneven terrain.
