29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift.      A) Exterior, in lift:  An employee was working in a Genie lift and was not tied off.  The employee was exposed to fall hazards of approximately 23 feet, on or about August 20th, 2019.

A) Exterior, in the lift under a balcony: An employee was exposed to head injuries while working below a balcony in a lift and was not protected by a hard hat or protective helmet, on or about August 21st, 2019.
