29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) On or about 08/19/19, on the jobsite of a house under construction at 970 Campbell Boulevard, Amherst, NY: A Ladder Jack Scaffold was erected on the northeast corner of the house. The scaffold had two support ladders, and the brackets that supported the aluminum pik were on the outside (the side away from the house) of the ladder. An employee used the scaffold to access & exit the roof. The employee stepped down from the roof onto the pik, climbed down and over the bracket on the east support ladder. A separate access ladder to the pik was not provided. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 08/19/19, on the jobsite of a house under construction at 970 Campbell Boulevard, Amherst, NY: One employee engaged in roofing work on the steep roof on the north side of the house was exposed to a fall of approx. 15 ft. because no fall protection was being utilized. Pitch of the roof was approx. 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED
