29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Units 114-116 (Southside Front of Structure): On or about August 21, 2019, the employer did not ensure that hard hats were used by employee(s) on the ground while worker(s) above approximately fifteen (15) feet on a ladder scaffold were performing vinyl siding installations; exposing employee(s) to struck-by hazards from overhead falling/flying materials and equipment.

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:  a) Units 114-116 (Temporary Power Pedestal Box: 200Amps, 240Volts Service): On or about August 21, 2019, the employer required employees to operate a live electrical panel box without a dead front where unprotected wires and bare connectors were present during construction activities; exposing employee(s) to electrical hazards.
