29 CFR 1910.178(l)(4)(ii): The employer did not provide refresher training to all employees engaged in the unsafe operation and use of forklifts:   At the facility located at 2525 Gholson Rd., Waco, Texas: forklift operators engaged in unsafe operation or involved in an accident/near-miss incident were not retrained to ensure they obtained the knowledge and skills necessary in the safe operation.  A forklift operator elevated an employee on the forks of a forklift approximately 8 feet to discard scrap metal and the employee was injured.  The employer is to provide retraining to the operator and any other employees engaged in operating or working around forklifts.
