29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.  a. On or about August 22, 2019, employees were exposed to a fall hazard up to 9 feet while installing trusses and suitable fall protection was not used.     b. On or about August 22, 2019, employees were exposed to a fall hazard up to 12 feet while accessing the roof and installing roofing material and a suitable fall protection system was not used.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:     On or about August 22, 2019, the employer did not maintain evidence of the required training for each employee who might be exposed to fall hazards.  There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
