29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about August 22, 2019, the employer failed to provide adequate guarding to protect employees from gaining access to the rotating chuck while operating the Clausing Colchester 15 Engine Lathe. Employees were exposed to amputation and laceration hazards while machining materials on the lathe.     Abatement documentation is not required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  On or about August 23, 2019, the employer failed to use an orange extension cord in accordance with its instructions in the listing or labeling.  The orange extension cord adjacent to the Clausing Colchester engine lathe was was plugged into a series of 3 different power strip adapters. This exposed employees to fire hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
