29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not updated and implemented for required respirator use:    Ensign Contracting and Restoration, LLC, Cumming, GA - On or about 08/23/2019, employees drilling/cutting/cutting concrete were provided and required to use a 3M half mask respirator. The written respiratory protection program did not include the program administrator, and respirators for Immediately Dangerous to Life and Health (IDLH) atmospheres.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     a)Ensign Contracting and Restoration, LLC, Cumming, GA - On or about 08/23/2019, employees were exposed to corrosive chemicals such as, but not limited to RMR 86 Pro Mold Stain & Mildew Remover, 35% Hydrogen Peroxide, MMR Mold Stain Remover, and Alkaline Cleaner/Degreaser, when the employer did not have a suitable facility for quick drenching or flushing eyes and body for immediate use in the shop.    b)Ensign Contracting and Restoration, LLC, Cumming, GA - On or about 08/23/2019, employees were exposed to corrosive chemicals such as, but not limited to RMR 86 Pro Mold Stain & Mildew Remover, 35% Hydrogen Peroxide, MMR Mold Stain Remover, and Alkaline Cleaner/Degreaser, when the employer did not have a suitable facility for quick drenching or flushing eyes and body for immediate use on the on jobs off site. 29 CFR 1910.37(b)(1): Each exit route was not equipped with operable lighting that to allow employees, with normal vision, to see along the exit route:  Ensign Contracting and Restoration, LLC, Cumming, GA - On or about 08/23/2019- In the warehouse area and office, employees were exposed to a fire hazard when emergency exit route lighting did not work. The employer failed to maintain and test the emergency lighting fixtures on a periodic basis to ensure the lights would function properly during a facility power failure.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
