On or about August 23, 2019, at the above addressed jobsite, employees working on the ground, below roofing activities, were not wearing a hard hat to protect against falling materials and debris.

On or about August 23, 2019, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet two (2) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
