Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by, fall, and/or caught between hazards associated with the installation of prefabricated roof trusses that were not adequately braced.   (a)  On or about August 20, 2019, and at times prior thereto, employees installed prefabricated 61' long span roof trusses weighing up to 1071 lbs. per truss, and the employer did not assure that the Temporary Installation Restraint/Bracing and the Permanent Individual Truss Member Restraint and Diagonal Bracing were installed properly and exposed employees to struck by, fall, and/or caught between hazards.  Note: Abatement certification AND supporting documentation ARE required for this item.

29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees:    a) On or about August 20, 2019, and at times prior thereto, employees used nail guns without safety glasses while performing framing activities.    b) On or about August 20, 2019, and at times prior thereto, employees working in locations where there was a risk of receiving head injuries did not don a protective helmet. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  On or about August 20, 2019, employees were exposed to approximate 18 foot fall hazards from exterior top plates and truss webbing while performing framing activities and setting trusses. 29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:  (a)  On or about August 20, 2019, and at times prior thereto, workers were engaged in framing activities and exposed to fall hazards of approximately 18 feet.  The employer did not communicate the hazards and/or have a fall protection plan to train employees in the use of fall arrest systems.
