Employees operated a Datsun 2700 sit-down forklift without adequate training.  Employees were exposed to struck-by hazards.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):    (a) Thirteenth Floor Entertainment Group, LLC DBA 13th Floor Haunted House at 3400 E 52nd Ave Denver, CO 80216:  On and before August 26, 2019, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v). Employees used chemicals including, but not limited to, gasoline, cleaners, resins, primers, and paints.  Secondary spray containers, such as the "Zep Professional Sprayer," contained unknown chemicals, and were not labelled with the correct information regarding contents.  This condition exposed employees to chemical hazards.
