29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury.  a) On or about August, 2019 at building # 2 on the fourth floor, room #421, an employee was using a Hitachi NT65M2 nail gun to attach base boards to the wall, without having eye protection.  b) On or about August 29, 2019, in the center of courtyard #2, an employee was using a Rigid R3205 7-1/4", circular saw to cut wooden planks without wearing safety glasses.

29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  a) On or about August 29, 2019 in the center of courtyard #1, employees were working in an JLG Aerial Lift (11411211) at an elevated height of approximately forty (40) feet to replace some panels on the building without fall protection.
