Employees of 720 Restorations LLC were reroofing a two story residential structure. The employees used chemicals during installation of the flat roof insulation and membrane. 720 Restorations LLC had not developed, implemented, or maintained a written hazard communication program. Hazardous chemicals at the work site included, but were not limited to the following: 1. Carlisle Construction Materials Weathered Membrane Cleaner; 2. Carlisle Construction Materials TPO Primer; and 3. Peak Building Products Shingle & Flashing Roof Accessory Paint.

Employees of 720 Restorations LLC used a portable right angle grinder with cut off wheel to cut stucco during re-roofing of a two story residential structure. Use of a handheld grinder for uses other than mortar is a task identified on Table 1. The employees operated the portable right angle grinder with neither a water delivery system nor with a dust collection system. The dry cutting operation generated dust. Stucco contains respirable crystalline silica. The employer did not fully and properly implement the engineering controls specified for the task on Table 1. This condition potentially exposed employees to a respiratory hazard.
