29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating: 1) Warehouse Area: On or about August 29, 2019, the employer exposed employees to struck-by hazard in that the holding latch used to secure the LP gas cylinder on the Toyota forklift truck model 5FCG30 and serial 5FCG30-70798, was missing/broken leaving the cylinder unsecured. 2) Warehouse Area: On or about August 29, 2019, the employer exposed employees to struck-by hazard in that the holding latch used to secure the LP gas cylinder on the Mitsubishi forklift truck model FGC250 and serial AF82B-0497, was missing/broken leaving the cylinder unsecured. 3) Warehouse Area: On or about August 29, 2019, the employer exposed employees to struck-by hazard in that the holding latch used to secure the LP gas cylinder on the Toyota forklift truck model 7FGCU25 and serial 82323 was missing/broken leaving the cylinder unsecured.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: 1) Warehouse Area: On or about August 29, 2019, the employer exposed employees to struck-by hazards in that the Mitsubishi forklift truck model FGC25 and serial AF82B-0497 that was not equipped with proper personal protective equipment such as a seat belt (operating restraint system) while employees are operating the equipment. 2) Warehouse Area: On or about August 29, 2019, the employer exposed employees to struck-by hazards in that the Toyota forklift truck model 7FGCU25 and serial 82323 that was equipped with proper personal protective equipment such as a seat belt (operating restraint system), but did not require employees to utilize the seatbelt when operating the forklift truck. 3) Warehouse Area: On or about August 29, 2019, the employer exposed employees to struck-by hazards in that the Toyota forklift truck model 5FCG30 and serial 5FCG30-70798 that was not equipped with proper personal protective equipment such as a seat belt (operating restraint system).
