29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 09/03/19 in the Blade Cell Department; employees were exposed to caught between hazards during the die change on hydraulic press 606-001 when the employer allowed the use of the emergency stop button and did not require the utilization of the SPX Lockout/Tagout Procedure found attached to the side of the equipment.    b) On or about 09/03/19 in the Mill Department; employees were exposed to rotating parts during the set up knives on the Cincinnati mill 405-01 when the employer allowed the use of the emergency stop button and did not require the  utilization of the SPX Lockout/Tagout Procedure found attached to the side of the equipment.   c) On or about 09/03/19 in the Blade Cell Department; employees were exposed to rotating parts during the set up knives on the Cincinnati mill 405-03 when the employer allowed the use of the emergency stop button and did not require the  utilization of the SPX Lockout/Tagout Procedure found attached to the side of the equipment.       ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation;   a) On or about 09/03/19 in the Blade Cell Department; employees were exposed to caught between hazards during set up and change over of Pacific hydraulic press 606-001, when the employer trained these employees as Affected and not Authorized employees.      b) On or about 09/03/19 in the Mill Department; employees were exposed to rotating parts during the set up knives on the Cincinnati mill (405-01) when the employer trained these employees as Affected and not Authorized employees.      c) On or about 09/03/19 in the Blade Cell Department; employees were exposed to rotating parts during the set up knives on the Cincinnati mill (405-03) when the employer trained these employees as Affected and not Authorized employees.      NO ABATEMENT CERTIFICATION REQUIRED
