29 CFR 1910.134(f)(1): The employer shall ensure that employees using a tight-fitting facepiece respirator pass an appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT) as stated in this paragraph.  (a) Pharmacy: The employer did not provide fit tests for all employees that are required to wear disposable N95 respirators when compounding hazardous drugs and participating on the hazardous drug spill team, on or about December 1, 2019.   Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.

29 CFR 1910.134(g)(1)(i): 29 CFR 1910.134(g)(1)(i): The employer permitted respirators with tight-fitting facepieces to be worn by employees who have facial hair that came between the sealing surface of the facepiece and the face or interfered with valve function:  (a) Pharmacy: The employer did not ensure there was no facial hair along the face seal for employees required to wear disposable N95 respirators when compounding hazardous drugs or are part of the hazardous drugs spill team, on or about January 27, 2020.   Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.
