29 CFR 1910.145(c)(1)(ii): Danger signs All employees shall be instructed that danger signs indicate immediate danger and that special precautions are necessary. On April 15, 2019, during the planned outage, subcontractor's employees were allowed to go up on the roof and breakroom area of the Steam Generator Building, elevation 217 ft. at Dynegy Zimmer Power Station, even with a "Danger, Keep Off Roof "sign on the door and were not instructed of the danger of a fire and explosion hazard and any special precautions when a hydrogen gas vent pipe line vented hydrogen gas during the de-gassing from the steam generator turbine.

29 CFR 1910.1200(h)(3)(iii): Training. The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used; and, On April 15, 2019, on the Roof and Breakroom area of the steam generator building, elevation 217 ft. Dynegy Zimmer Power Station, the employer's safety orientation to contractor personnel, their hazard communication program's method of informing contractor personnel of the precautionary measures that need to be taken., did not inform Enerfab contractor employees of the hazards and precautions from the de-gassing of hydrogen gas from the steam generator turbine that was occurring that day and was venting from the hydrogen gas vent pipe line located on the roof area of the steam generator building, elevation 217 ft., the area where the contractor employees were being located for staging equipment, tools and personnel.
