Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) On August 29, 2019 at the address of 807 Shawnee Road, Lima OH: the employer did not assure employees were protected for fall hazards while removing roofing material from a flat commercial roof.  The employees were exposed to a 15 foot fall to the ground below.
