29 CFR 1926.100(a): The employer did not ensure that employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were protected by protective helmets.  This violation occurred on September 5, 2019, where multiple employees working below employees working on decking above were exposed to being struck by falling materials and/or tools from more than 17 feet above.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, the employer did not ensure that each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.  This violation occurred on September 5, 2019, where multiple employees installing the metal deck were exposed to falls of more than 17 feet to the concrete below.  No fall protection in place.
