29 CFR 1926.701(b): All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:    On or about 05 September 2019, and at times prior thereto, employees worked around a manhole where vertical rebar was installed. The 1/2 inch rebar  was ~37.5 inches tall and not capped, exposing employees to the hazard of being impaled by the rebar.

29 CFR 1926.1053(b)(4): Ladder was not used for the purpose for which it was designed.    On or about 05 September 2019, and at times prior thereto, employees used a fiberglass step ladder as means of entrance and egress into a trench instead of an extension ladder. The ladder did not extent three feet past the landing surface, thus exposing the employees to the hazards of slips, trips and falls due to the unstable ladder and insufficient length.
