(a)  Claster Blvd and Floral Lane Dauphin PA - Employees entered a trench approximately 8 feet deep that was not provided with required benching, sloping, or other engineered protective systems exposing employees to hazards related to a trench collapsed, on or about September 6, 2019.     No abatement certification or documentation required.
