29 CFR 1926.451(a)(1): Except as provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section, each scaffold and scaffold component shall be capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.   a. Located at 1605 Dickson in Marysville, Ohio: On or about September 6, 2019, employees were performing siding work from a pump jack scaffold system. The scaffold platform was extended approximately four foot on one end by strapping a two inch by twelve inch wooden board to the platform and approximately three foot on the other end by strapping an aluminum plank to the platform. The employer did not ensure that the modifications to the scaffold platform were approved and capable of supporting the load applied, thereby exposing employees to an approximately fourteen foot fall hazard.

29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.   a. Located at 1605 Dickson in Marysville, Ohio: On or about September 6, 2019, the employer did not ensure that a competent person was onsite while the pump jack scaffold was erected that was qualitied to ensure that the scaffold was erected properly. Employees were exposed to fall hazards while working from an unsafe scaffold platform approximately fourteen foot from the ground without effective means of fall protection in place and improper planks attached to the scaffold platform to extend the platform's width.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.   a.
