29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about September 06, 2019, at the worksite located 4230 Summit Knoll Drive, Saint Louis, Missouri, 63129,  employee(s) for Luis Zavala-Velasquez were observed using a portable extension ladder to access the roof without extending the side rails at least 3 feet above the upper landing. The fall distance to the ground below was approximately 19 feet.

On or about September 06, 2019, at the worksite located  4230 Summit Knoll Drive, Saint Louis, Missouri 63129, employee(s) for Luis Zavala- Velazquez were observed installing shingles on a multi-family, residential dwelling without the use of a guardrail systems, safety net system, or personal fall arrest system.  The fall distance to the ground below was approximately 19 feet.
