29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  JOB SITE: 9200-9300 Spring Creek Lane, O Fallon, MO  63366, on or about 9/6/2019, workers were observed on to the roof, operating pneumatic nail guns, actively installing new roof material, without any form of eye or face protection.  The workers were exposed to struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures  JOB SITE: 9200-9300 Spring Creek Lane, O Fallon, MO 63368, on or about 9/6/2019, workers were observed on top of a 8:12 pitch roof, actively installing new roof material, approximately 18 feet, 4 inches eave to ground level, without fall protection.  The workers were exposed to a fall hazard.
