29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly:  (a) At the workplace: On or about September 6, 2019, a 6-ft long carriage mounted carpet pull/rug-ram attachment was used with the Yale Lift Truck Model GLP050VXEURE086 without the manufacturer's approval.  (b) At the workplace: On or about September 6, 2019, the instruction plates on the Yale Lift Truck Model GLP050VXEURE086 were not changed to account for the use of the 6-ft long carriage mounted carpet pull/rug ram attachment.
