29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a) Site on floors 2-4 at staircase landing: On or about September 6, 2019: the employer exposed employees to fall hazards in that, of 8" floor holes were not covered while employees were installing wallpaper at the worksite.

29 CFR 1926.405(a)(2)(ii)(E): Lamps used in temporary wiring for general illumination were not protected from accidental contact or breakage:  (a) Site: On or about September 6, 2019: the employer exposed employees to electrical shock hazards in that, lamps in the hallway was not adequately protected from accidental breakage while contractors were accessing the staircase to perform punch out work.
