29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about September 9, 2019, on the 5:12 pitch roof of building 60, 4 roofing employees tearing off old roofing materials were exposed to an 18 feet fall hazard in that they were not protected by a fall protection system.

On or about September 9, 2019 at the northwest corner of the roof eave of building 60, the roof access ladder did not extend 3 feet above the roof eave, exposing 4 roofing employees to an 18 feet fall hazard.
