29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a)  751 Riverside Dr. Lot 367, front Roofs: On or about 09/10/2019 the employer exposed employees to fall hazards of up to 20 feet from ground level while they were performing siding installation from the second story of a 4 in 12 pitch roof on a new residential construction home without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) 751 Riverside Dr. Lot 367, front Roofs: On or about 09/10/2019 The employer exposed an employee to falls of 20 feet to the ground below while using a Werner ladder to access the second story roof that only extended 1.5 feet above the landing. The ladder was not fastened at the top to prevent tipping.

(a) 751 Riverside Dr. Lot 367, front Roofs: The employer exposed employees to fall hazards of 20 feet while performing work on a 4 in 12 pitch roof when certification of fall protection training was not completed.
