29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   (a) 125 Hawkins Dr. Lot 112, Scaffold: On or about 09/10/2019 the employer exposed employees to struck-by hazards when employees were allowed to walk under the raised pumpjack scaffold without hard hats on while work was being performed overhead. They were exposed to falling objects such as pneumatic nailers or siding.

29 CFR 1926.451(h)(2)(i): The area below the scaffold was not barricaded, and employees were not prohibited from entering the hazard area.  (a) 125 Hawkins Dr. Lot 112 by scaffold: On or about 09/10/2019 the employer exposed an employee to struck-by hazards in that the employee was allowed to walk and work in areas under and around the pumpjack scaffold because the scaffold was not barricaded at the bottom.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   (a) 125 Hawkins Dr. Lot 112, Scaffold: On or about 09/10/2019 the employer exposed employees to falls of 20 feet to the ground in that they were using an Alum-A-Pole scaffold to install siding without guardrail systems installed along all open sides and ends of platforms.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.   (a) Side of building beneath the Pumpjack scaffolding: On or about 09/10/2019 the employer exposed employees to fall hazards in that they allowed two employees to perform siding installation 20 feet above ground level from an Alum-A-Pole scaffold without the poles of the scaffold being secured to the structure by rigid triangular bracing or equivalent at the bottom.
