29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.    On or about September 11, 2019, on the 5:12 pitch roof, 3 roofing employees were exposed to an 18 feet fall hazard from the roof eaves in that they were not protected by a fall protection system.  b.    On or about September 11, 2019, on the south side deck, 3 roofing employees who used the extension ladder from the deck were exposed to an 11 feet fall hazard in that the deck railings had been removed, and the employees were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    On or about September 11, 2019, at the south eave, the roof access ladder extending from the deck to the roof was not extended 3 feet above the roof eave, exposing 3 roofing employees to a 7 feet fall hazard.

29 CFR 1926.1053(b)(9): The area around the top and bottom of ladders was not kept clear:  a.    On or about September 9, 2019, on the south deck, the area around the base and the top of the roof access ladder from the deck to the roof eave was cluttered by discarded roof tarpaulins and loose roof shingles, creating trip and slip hazards to 3 employees who used the ladder.
