29 CFR 1926.451(f)(13): Debris was allowed to accumulate on scaffold platforms: a) On or about 09/11/19, in the front of the building of 68-64 Yellowstone Blvd. Forest Hills, NY Debris, such as loose rocks, bags, and sheets were strewn on scaffold platforms. Employees working on the scaffold platforms were exposed to tripping and falling hazards approximately 60 ft. to the ground. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) On or about 09/11/19, in the front of the building of 68-64 Yellowstone Blvd. Forest Hills, NY Employees working from a scaffold platform, approximately 60ft above the ground, were not protected from falling. ABATEMENT DUCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
