On or about September 11, 2019, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about September 11, 2019, at the above addressed jobsite, employees performing residential roofing operations, ten (10) feet above the ground were not protected from falls, thereby exposing employees to fall hazards.  Peak Construction & Exteriors Corp. was previously cited for a violation of this Occupational Safety and Health Standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1262253, citation number 1, item number 1, and was affirmed as a final order on October 16, 2017, with respect to a workplace located at 141 - 149 W. Farm Gate Drive, Schaumburg, IL 60159.  Peak Construction & Exteriors Corp. was previously cited for a violation of this Occupational Safety and Health Standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1257312, citation number 2, item number 1 and was affirmed as a final order on November 30, 2017, with respect to a workplace located at 14 Carrington Ct, Hazel Crest, IL, 60010.  Peak Construction & Exteriors Corp. was previously cited for a violation of an equivalent Occupational Safety and Health Standard  29CFR 1926.501(b)(10) which was contained in OSHA inspection number 1097235, citation number 1, item number 1 and was affirmed as a final order on April 6, 2016, with respect to a workplace located at 11100 S. Indiana Ave., Chicago IL 60605.
