29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. Site - Employees performing roofing work were exposed to falls of approximately 14 feet when the ladder used to gain access to and from the roof was not extended at least three feet above the roof surface.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a. Site - Employees performing roofing work were not protected from falls of approximately 10 feet when fall protection was not used or provided.       Trancis Fast Construction Corporation was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1203863, citation number 001, item number 001, and was affirmed as a final order on June 6, 2017, with respect to a workplace located at 357 Central St. Manchester, NH 03101.
