29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   (a)  On or about 09/06/19, at the Canandaigua National Bank, under construction, located on 6660 Fourth Section Road, in Brockport, New York, employees working in the basket of an extensible boom platform (JLG aerial lift), were not wearing a harness with lanyard to "tie-off" to the basket of the JLG aerial lift.  The employer did not enforce fall protection, where the employee was exposed to being ejected out to the boom basket.  NO ABATEMENT CERTIFICATION REQUIRED
