On or about September 13, 2019, the employer did not ensure that an employee working in the JLG Aerial Lift was trained to operate the equipment, exposing the employee to a fall hazard of up to 6 feet fall to the ground.  b.  On or about September 13, 2019, the employer did not ensure that an employee working from a scaffold was trained to work from the scaffold, exposing the employee to a fall hazard of up to 5 feet fall to the ground.
