29 CFR 1926.760(a)(2): On multistory structures, perimeter safety cables were not installed at the final interior and exterior perimeters of the floors as soon as the metal decking was installed:    (a) 150 Hain Avenue, Lebanon, Pennsylvania - Employees worked erecting chicken egg production equipment, which comprised a steel structure, including walking/working platforms and chicken cages, for a new chicken egg-laying house.  Employees had finished installing the metal decking on the top level but did not install perimeter safety cables at the final interior and exterior perimeters on the south end of the chicken egg-laying house, exposing employees to a fall hazard, on or about September 17, 2019.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:  (a) 150 Hain Avenue, Lebanon, Pennsylvania - Employees erecting the chicken egg-laying equipment, which comprised a steel structure, did not have training on steel erection fall protection requirements, exposing the employees to a fall hazard, on or about September 17, 2019.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:    (a) 150 Hain Avenue, Lebanon, Pennsylvania - The landing, 8 feet 7 inches above the ground, on the stairway at the south end of the chicken egg-laying house under construction, had no guardrail system on either open side.  Employees used the stairway to access above ground levels, exposing employees to the hazard of falling to a lower level, on or about September 17, 2019.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
